Claiming for Lost & Found Items, Seized Locker Items (http://pshs.edu.ph/)

Office of the Registrar
CLAIMING PROCEDURE FOR LOST & FOUND ITEMS,
SEIZED LOCKER ITEMS
 
1. Per our Student’s Handbook for SY 2010-2011, lost-and-found items unclaimed “after the last day of classes in November and March shall be donated to charitable institutions...” (however, please note that items lostbefore but unclaimed as of November 15, 2010 had already been donated to such institutions through the Student Alliance.)
2. Meanwhile, items seized from lockers are also in our possession.
3. To date, we still have not donated such items surrendered after November 15, 2010 until March 31, 2011.
4. We are giving students until 4:00PM, April 18, 2011 to look through all the items and claim them.
5. Students claiming any item must fill out the Claim Form (available at the Office of the Registrar) and signed by one of his/her parents.
6. All the items will be disposed of on April 19, 2011.
7.  No more extensions shall be granted.

CARD-GIVING PROCEDURE for March 31, 2011
1. The student must present his fully accomplished clearance form to the Registrar staff in-charge of distributing the report card to claim his/her report card. NO REPORT CARD WILL BE ISSUED to students with INCOMPLETE clearances/UNCLEARED items.
2. If you have accountabilities at the Office of the Registrar, you shall not be entertained on March 31, 2011. Please accomplish your accountabilities with the Registrar’s Office from May 23-27, 2011.
3. If you missed your photo session schedule without prior notice to the Registrar’s Office, you will be accommodated only on June 1, 2011 onwards. Each day of non-wearing of the ID shall be one non-compliance. (This is a reiteration of our previous announcement regarding pre-enrolment procedure.)
4. Students with substandard grades (2.75 or 3.0) must be accompanied by his/her parent or guardian for SY 2011-2012 must be accompanied when claiming the card.
5. Students who are not able to claim their report card on 31 March 2011 may do so on May 30-31, 2011, provided the clearance is fully accomplished and presented to the Registrar staff in-charge.
6. No report cards will be issued during regular enrolment, the schedule of which will be announced later.
7. The report card is a requirement for enrolment.
8. Please be guided by the above procedure.

Announcements/Clearance Requirements

Message from the SSD Office: Official time for March 21(Mon) & March 22 (Tues) shall be until 12 noon or after finishing activities, e.g. checking clearance requirements from teachers, admin units (except those joining the bio? field trip). Gate pass no longer needed because the 12 noon is official dismissal time. Please advise your parents accordingly.

Clearance requirements from the Batch Adviser:
For everyone:
(1) payment of all unpaid dues (concert tickets, class funds, outbound, prom pics, batch shirt)
(2) scrapbook page which contains your original (not scanned) prom picture with prom formal picture background and some information about you
(3) accomplished outbound worksheet for those who did NOT join the outbound - get the worksheet from me on monday 9AM
(4) thank you note to their generous benefactors for ALL those sponsored in previous batch activities (retreat, prom, outbound).

For class treasurers: list of those with unpaid class funds (I WILL NOT CLEAR THEM ALSO)

For class presidents: compiled complete (no one is missing) scrapbook. SUBMIT BY TUESDAY TO THE REGISTRAR, I WILL NOT BE AROUND ON TUESDAY SO AS LONG AS YOU SUBMIT YOU WILL BE CLEARED. OJ: Batch 2012, one member in your class has been given a note and a copy of the Cadet Form for next year. Additional notes: Only black ink should be used & you should put your whole name in both the left side and the right side. Even Medic applicants will have to fill up the form. You may submit your forms on Monday or next year. If you will submit it on Monday, there is no need for a 1x1 picture. For next year, the picture you should put there should be recent (over the summer taken) & dapat red yung background. Thank you.-gm-

I. Photo Session for School ID
a. All students must come well-groomed and in proper uniform. Males must be in proper haircut (2” x 3”) and Females sporting long hair should tie their hair.
b. The photo session for the new school ID shall be according to the set schedule shown below. The ID must be worn starting June 1, 2011.
c. If any student misses his/her schedule, he/she will be accommodated only on June 1, 2011 onwards. Each day of non-wearing of the ID shall be one non-compliance.
d. Venue shall be at the 2nd Flr Seminar Room.

The final BaCon has been scheduled for tomorrow, after the CEM examinations. Part of the agenda are the outbound orientation and career fair sign-ups. Attendance is mandatory.

All frog dissections scheduled for tomorrow are moved to Wednesday, March 16.

Field Trip Shiz

Guys, for the field trip sabi ni sir nat, we should be wearing jogging pants/jeans tapos shirt (better if batch shirt).


Bring extra clothes because there is a pretty good chance there will be wading/we will get wet.

Baka mainit so bring an umbrella :> *hint hint* (joke lang =)))

Also, rappelling will be free, although you will need to have a waiver/parent's consent to be allowed to join. (And this may or may not push through depending on the time constraints. so kung natuloy man, i suggest you line up kaagad.)

Frog Dissection

We’re having this on the Tuesday (Mar 15th) after the Perio.

  • Cs will do it at 730-830AM,
  • Li at 830-930AM,
  • Mg at 930-1030AM,
  • Na at 1030-1130AM.

Let me know if you have problems with the schedule.

Per group, kindly prepare:

  • 1 live frog or toad (can be obtained from UP NSRI @ around PhP50-70/frog) — please submit these to me on the morning of dissection day
  • gloves (if you’re squeamish)
  • soap
  • alcohol

As part of the prelab activity, please check out this virtual frog dissection link that will show you how it’s done. You can download the prelab sheet and procedure here. If you have any ethical or personal concerns regarding any part of frog dissection: the use of live frogs, pithing (see second definition), etc., please leave a comment so we can plan around them.

FAIR BOOTH!

Name: Nobody, Nobody But You! (Karaoke Booth)
Description: The booth will be a singing-showdown catcher booth. Sort of American Idol style with 3 judges (can be less), and 2 captured "contestants" who will sing a song picked by the judges. The judges will score the contestants on their singing (be impartial please, judges) and the contestant with a higher score goes free. The loser shall get flushed (dumped on with water), or he can ask for another chance against a different contestant. If he loses again, he gets a worse punishment (flour with water).
Size/Area: Somewhere with a plug for speakers. But near-ish the oval since basaaan.
Materials: 2-3 microphones, speakers, buckets, laptop, FLOUR.
Budget: We will try to borrow from the class to minimize money loss.
Pricing: Contestants can pay P50 to bail from singing. (AFTER SINGING, BAIL IS STILL ALLOWED. BUT IS RAISED TO P100. )
People in the audience can pay P20 to request a song for the contestants to sing.
People can request catches for P50.
Promotion: we have posters na! (c/o bianchi)
